When should constipation be alarming after surgery?

i had surgery 5 days ago and i havent been able to pass a good bowel movement . i already have took two stool softener and a suppository and havent been able to do nothing. im in so much pain its hard to breath will my doctor be able to help ? has anyone had this problem.
