5 year old with hemroids ?

My 5 year old and his twin brother have both been suffering severe watery diarrhea and have been constipates for 14 days now!! laxatives didn't work so tried suppository's this worked for one but it just mad my other son bleed, he is complaining of a sore itchy bottom and says when he goes poopy he can feel something going in and out, i had a look up his bottom (i'm a nurse i know what i'm doing) what can i do to help him ????
